Referrals & Collaborative Care

Clinical Support for Occupational Therapists, Physicians & Case Managers

Family physicians, nurse practitioners, occupational therapists, case managers and specialists refer to us for psychological assessment and treatment, trauma therapy, physiotherapy, nutrition support, TMS and neurofeedback, and catastrophic impairment determination.

Motor vehicle accident care carries an administrative burden that sits poorly alongside a busy practice. We take that on, and we report back to you in a form that is useful in a ten-minute appointment.

We are a member organization of the West Toronto and Mississauga Ontario Health Teams, and provide counselling under Urgent Response Services.

What We Offer

How We Work With You

Four different working relationships, depending on what your patient needs.

Straightforward Referral

Send us the patient and we take it from there: assessment, treatment plan, insurer funding request, and treatment. You receive confirmation of receipt, the assessment findings, and progress reports.

No forms of ours to complete. A note or a phone call is enough to start.

Consult Before Referring

Where you are unsure whether referral is appropriate, or which discipline is needed, call us first. We will give you an opinion without requiring a formal referral.

There is no charge for that conversation, and we will tell you if we are not the right service.

Co-Management

For patients you intend to continue managing, we work alongside you rather than taking over. You retain primary care, we deliver the specialist components, and we report at agreed intervals.

We will not change medication or make recommendations outside our scope without discussing it with you.

Transfer of MVA-Related Care

Where a patient’s accident-related needs have grown beyond what a general practice can reasonably absorb, we can assume responsibility for the accident-related components entirely.

That includes the insurer correspondence, funding requests, treatment planning and multidisciplinary coordination, while you continue with their general medical care.

Specialist Opinions, Coordinated

Where a patient needs a medical specialist opinion, we coordinate that as well, including psychiatry, orthopaedic surgery, neurology, physiatry, ENT, neuro-optometry, and chronic pain medicine. We arrange each appointment and consolidate the findings, so you receive one coherent picture rather than separate reports arriving at different times.

Bedside Attendance Before Discharge

Where a patient is still an inpatient, our counselling team can attend at the bedside. Beginning support before discharge means the patient leaves hospital already engaged in treatment rather than joining a waitlist.

Our clinics inside Oakville Trafalgar Memorial Hospital and the Marotta Family Hospital make this straightforward, and we attend at other sites across the region on request.

Referral Pathways

What You Can Refer For

Tap any service for what it involves and when it is indicated.

Psychological Assessment & Treatment

Psychodiagnostic assessment by registered psychologists for clients aged 18 and over, with diagnosis against DSM-5-TR criteria supported by standardized psychometric measures.

Treatment follows directly from the findings, delivered by registered psychotherapists, psychologists, social workers and nurse practitioners. Indicated where a patient presents with PTSD, anxiety, depression or adjustment difficulty following a collision, or where diagnostic clarity is needed before treatment.

Trauma Therapy & Counselling

EMDR, cognitive processing therapy, prolonged exposure, CBT and more than thirty other modalities across the team, for ages four and up.

Particularly useful for patients whose symptoms have not responded to general supportive counselling, since trauma frequently requires trauma-specific treatment.

Neuropsychological Assessment

Standardized cognitive testing where brain injury is suspected or post-concussive symptoms persist, scored against demographically corrected normative data.

Findings distinguish injury-related impairment from the effects of pain, disrupted sleep, medication and mood, which is often the clinical question you actually need answered.

Physiotherapy

Whiplash, spinal pain, headaches, post-concussion presentations, chronic pain and deconditioning, with movement and strength measured using FDA-cleared devices rather than estimated.

Available in Grimsby and St. Catharines, and by home visit across Hamilton, Niagara and the surrounding areas.

Registered Dietitian

For digestive injury following seatbelt or abdominal trauma, jaw and TMJ pain limiting oral intake, appetite loss secondary to medication or mood, unintended weight change, and nutritional support for tissue healing. Ages six and up.

Also useful where an existing condition such as diabetes has become harder to manage since the injury.

TMS for Depression

Transcranial Magnetic Stimulation at our Oakville clinic, for depression that has not responded adequately to pharmacological and psychological treatment.

A reasonable next step before, or alongside, further medication trials in treatment-resistant presentations.

Neurofeedback & Biofeedback

Available at our Oakville clinic, for anxiety, attention difficulties, sleep disturbance and autonomic dysregulation.

Also used therapeutically alongside our Mental Health Driving Anxiety Assessment©, where physiological response is measured directly.

Catastrophic Impairment Determination

We coordinate the full multidisciplinary assessment, including orthopaedic surgery, neurology, psychiatry, physiatry, neuro-optometry and chronic pain specialists, and conduct the psychological and functional components in-house.

You receive one integrated report rather than a set of disconnected opinions, and your patient is not left arranging specialist appointments while unwell.

Specialized Assessments

Three assessments developed in-house and available through no other provider in Ontario: driving anxiety measured using FDA-cleared virtual reality and biofeedback, The Quantitative Sleep Assessment©, using an FDA-cleared device across three or more nights at home, and the Functional Impact and Barriers to Recovery Assessment© conducted by a Registered Social Worker.

Each produces objective data where the presentation would otherwise rest on self-report alone.

For Occupational Therapists

A Referral Partner for Occupational Therapists

We provide the psychotherapy, social work, psychology, psychiatry, and dietitian services your motor vehicle accident clients need alongside occupational therapy. Occupational therapy, functional capacity evaluations, in-home assessments, and Future Cost of Care reports fall outside our scope. We exist to support that work, not to compete for it.

When Your Occupational Therapy Services Are Denied

This is the part most referral partners will not do. When an insurer denies or cuts your occupational therapy hours, we respond directly to the adjuster in writing, setting out the clinical necessity of occupational therapy on that file and attaching the mental health and functional findings that substantiate it.

We know what occupational therapy contributes to a recovery and to the clinical record, and we are willing to say so on the record. You should not have to make that case alone.

Documentation Written to Strengthen Your Reports

Our clinical records are structured to complement occupational therapy assessments rather than sit apart from them. We document mental health, behavioural, cognitive, and nutritional findings using objective measures and real-world functional examples, so the psychological picture is recorded in the same functional language your reports use.

We document impairment across the four spheres of function, activities of daily living, social functioning, concentration and pace, and adaptation to stress, and we align our records with the AMA Guides and SABS standards where applicable.

Our Clinical Narrative Reports give an OT team the mental health and nutritional context needed to support functional capacity assessments, in-home assessment reports, and Future Cost of Care recommendations. We do not produce Future Cost of Care reports ourselves, and we are available for consultation to the assessor who does, to clarify findings and make sure the full scope of impairment is accurately reflected.

One Consistent Record Across Every Discipline

Psychiatry, psychology, psychotherapy, social work, and dietetics all contribute to a single coherent clinical narrative rather than separate files that contradict one another. Gaps and inconsistencies between providers are the first thing an insurer looks for, and a shared record removes them.

Notes are concise, objective, and focused on impairment. We go beyond diagnostic labels to record the specific functional limitations, behavioural changes, and daily difficulties that demonstrate what the injury has actually cost the client.

Nurse Practitioner Access to Specialists and Diagnostics

Our nurse practitioners expedite referrals to OHIP-funded specialists and specialised programmes, including outpatient traumatic brain injury programmes, and order diagnostics such as nerve conduction studies, MRI, and CT.

For an OT, this closes the evidentiary gaps that otherwise sit unresolved for months while a client waits on a family physician referral, and it means the medical record catches up with what you are observing functionally.

Social Work: Government and Community Navigation

Our Registered Social Workers connect clients to the supports that are otherwise left to whoever has the closest relationship with the client, which is frequently the OT. That includes Ontario Works, ODSP, CPP Disability, Employment Insurance sickness benefits, Disability Tax Credit and Canada Caregiver Credit certification, rent-geared-to-income and subsidised housing, supportive and transitional housing, Home and Community Care, Assistive Devices Program, Trillium Drug Program, Legal Aid navigation, Victim Services, employment supports and retraining, food security resources, Indigenous services, and settlement services for newcomers.

We are also an approved provider under the Interim Federal Health Program, administered by Medavie Blue Cross for Immigration, Refugees and Citizenship Canada, so we can serve refugees, refugee claimants, and protected persons, including those without provincial health coverage, and we handle the eligibility and claim submission ourselves.

The practical effect is that the navigation work comes off your desk, and the unmet need identified along the way strengthens the social functioning and daily living evidence on the file.

When Funding Stalls, Care Does Not

We prepare the clinical justification responses that challenge OCF-18 denials. Where a plan is denied or delayed, we do not let the client sit idle: we look at extended health benefits to bridge access in the interim, and we offer protected accounts and third-party financing through MediCard and BridgePoint Financial.

Cost should not decide whether a client keeps progressing, and a treatment gap in the record helps nobody.

Capacity After a Catastrophic Determination

Once catastrophic impairment is determined, the intensity and coordination required change considerably, and many providers cannot scale to it. We can. Our team delivers the sustained multidisciplinary treatment required post-determination and continues building the clinical record that supports ongoing entitlement and settlement discussions.

We also coordinate and complete the multidisciplinary catastrophic assessment itself, including the psychological and functional components, with medical specialists arranged and findings consolidated into one integrated report.

Assessments Occupational Therapy Alone Cannot Capture

Biopsychosocial assessment by a Registered Social Worker or psychotherapist, documenting pre- and post-accident function across physical, psychological, social, and occupational domains.

Three proprietary, copyright-protected assessments available nowhere else in Ontario: the Mental Health Driving Anxiety Assessment©, measuring driving and passenger anxiety objectively using FDA-cleared virtual reality and biofeedback rather than interview; The Quantitative Sleep Assessment©, recording sleep physiology over a minimum of three nights in the client’s own home with an FDA-cleared device and grading the severity of daytime impairment; and the Functional Impact and Barriers to Recovery Assessment©, examining nine barrier domains that standard reporting routinely leaves undocumented, recommended at around eighteen months.

Also available: psychological assessment with DSM-5-TR diagnosis, completed virtually, our Neuromuscular and Gait Assessment using FDA-cleared inertial motion analysis and handheld dynamometry, and nutrition assessment by a Registered Dietitian on our team. Where the client consents, assessments include a collateral interview with family or caregivers to corroborate reported limitations.

Where and How We See Your Clients

Four clinics: Oakville inside Oakville Trafalgar Memorial Hospital, St. Catharines inside the Marotta Family Hospital, Grimsby, and Kitchener. In-home and in-community psychotherapy and social work throughout the Greater Toronto Area, Hamilton, Niagara and Waterloo Region. Virtual appointments across Ontario. Dietitian services in person in the Niagara and Hamilton regions and virtually across Ontario.

Interpretation is available on demand in over 300 languages at no additional cost to the client, and where travel is the obstacle we can arrange transportation.

Clients are seen from age four. There is no waitlist, and for accident benefits clients the insurer has ten business days to respond to a funding request, though on a case-by-case basis we may begin sooner.

To discuss a client, arrange a coordinated intake, or introduce your team to ours, call 289-547-5911 Toll Free 1-866-248-0567 or email info@criticaltraumatherapy.com. There is no cost to a conversation, and no form of ours to complete.

Reporting

What Comes Back to You

A referral that produces silence is worse than no referral at all.

Acknowledged Promptly

You receive confirmation that your patient has been received, and who will be seeing them.

Findings You Can Use

Assessment reports state the findings plainly, set out the reasoning, and answer the clinical question you asked. Test data supports the opinion rather than substituting for it.

Written to be read in the time a real appointment allows.

Progress at Intervals

Structured progress reporting as treatment proceeds, with objective re-measurement rather than impressions. If attendance lapses or the presentation changes materially, you hear it from us.

Reporting is monthly for patients in active treatment, and material changes are communicated as they arise rather than held for the next cycle.

Records on Request

Complete records released promptly, transferred securely, and provided in chronological order so the file arrives ready to review.

Making a Referral

How to Refer

Call us on 289-547-5911, email info@criticaltraumatherapy.com, or fax 289-426-3227. A brief note is sufficient, and we will contact your patient directly to arrange assessment.

Please include what you are asking us to address, any relevant history, and whether an insurer or lawyer is involved. If you have imaging or reports, send them, but their absence will not delay us.

Where funding is through accident benefits, we complete the OCF-18 and deal with the insurer. Nothing further is required from your office.
